For each type of fund offered by the foundation, the community foundation submits a sample fund agreement that contains the terms and conditions for each fund.
Material Change: Any changes in sample fund agreements must be submitted. In addition, if the foundation offers new types of funds, sample fund agreements must be submitted for any new types of funds. If any changes were made, please submit the revised documents and fill in chart below where appropriate.

Donor advised fund guidelines contain a complete description of all related activities that affect donor advised funds. These include communicating with donor advisors, restrictions on donor advised funds and donor activities, and processes related to donor recommendations and naming successors.
Tab 27: Documentation of Procedures Ensuring that Funds Are Used to Meet Donors’ Intent
The community foundation submits the process used for determining and documenting donor intent and the process for considering and approving grants consistent with donor intent.
Tab 28: Evidence that Fund Statements Are Available
The community foundation should demonstrate that fund statements are provided, and show the frequency of fund statement distribution.
Tab 29: List of Funds
The community foundation submits a list of funds, organized by fund type (e.g., scholarship, designated, donor advised). If a community foundation has more than 200 funds, a sample of 25 funds for each fund type will be sufficient and for those foundations with fewer than 200, the list provided should be comprehensive.
